Form Usage
Use the Complaint and Appeal form to submit an official academic/administrative complaint or appeal. A link to the form can be found on the Academic Affairs workflow site at https://workflow.twu.edu/AssocProvost/_layouts/15/start.aspx#/SitePages/Home.aspx

*Note: During testing, a temporary link to the test form will be available. This link will be removed and replaced with the production link once we go live.
On the form page, click Academic/Administrative Complaint and Appeal Form to open the form. You will be prompted to log in. Enter your Portal username and password to log in. The system will configure your account settings, then take you to the form itself. This process may take up to a full minute to complete.
*Note: During testing, you will be able to log in and your information will be replaced by a student’s information for testing purposes. Let me know when/if you would like to change which student account is used for testing.)
Review your Student Information carefully. If any changes need to be made, please contact the Registrar’s office to correct your information before submitting this form.
Fill out the form by choosing the Date of Occurrence, that is, the date of the event that is the subject of this complaint. Then choose the Topic of Complaint from the drop down menu. If your complaint is a grade appeal, a drop down menu will appear where you can select the Course Affected. Finally, describe your complaint in the Complaint/Appeal section, and provide a description of what action you expect to be taken in The Request section.
When you are done, click the Submit button. An email receipt will be sent to you.
