This short form is used for (Full-Time Equivalency) FTE adjustment requests for increase or decrease. Discuss the FTE adjustment with your next level of authority, complete the form and route the form through the applicable signatures. This form can be used for both permanent FTE adjustments and temporary FTE adjustments. Once it reaches the Compensation Team we will send you an email notification when approved.
This form does not take the place of the Manager-Self Serve (PTF). Your department will still need to process the Manager Self-Serve (PTF) to make the change in the system for your employee(s).
