This short form is for both Academic and Non-Academic Departments that need to change an existing department name. Use this form to make department name changes. Complete the form and route the form through the applicable signatures. Once it reaches the Compensation Team we will build the new department name. We will then send you an email notification when approved.
Using this form helps notify everyone that needs to be involved with these types of changes, i.e. the Department Heads, the Budget Office, Provost/VP’s and HR.
