Americans with Disabilities Act Accommodation Information

An employee with a disability who seeks an accommodation in order to perform the essential functions of the job may initiate the accommodation process by making a verbal or written request to the Office of Human Resources, ADA Coordinator or their immediate supervisor.

To be protected by the ADA, one must have a disability, which is defined by the ADA as a physical or mental impairment that substantially limits one or more major life activities, a person who has a history or record of such an impairment, or a person who is perceived by others as having such an impairment. The ADA does not specifically name all of the impairments that are covered.

All requests for workplace accommodations are reviewed on a case by case bases and the documentation guidelines are implemented at the discretion of the ADA Officer.

ADA Accommodation Requests:

ADA accommodation requests involve the following steps depending on whether you are Faculty/Staff Employee, Supervisor or Job Applicant.

Faculty/Staff:

1. Faculty and Staff employees may submit an ADA accommodation request directly to the Office of Human Resources. Please contact the ADA Coordinator, Rebecca Gardmer at 940-898-3759 or rgardner2@twu.edu. 
2. The ADA Accommodation Request form must be completed and submitted to initiate an ADA accommodation request. The Medical Inquiry Form may also be downloaded and submitted to your medical provider. Both completed forms may be submitted to Rebecca Gardner at rgardner2@twu.edu. 

Supervisors:

1. When a department receives an accommodation request from either an employee or an applicant, the supervisor or hiring official should review TWU’s URP: V.17.d Americans With Disabilities Act Policy and/or the ADA Hiring Guide for Employment.
2. Once the policy has been reviewed and the supervisor/hiring official has an understanding of what the employee/applicant may be requesting as an accommodation, the supervisor/hiring official should contact the ADA Coordinator, Rebecca Gardner in the Office of Human Resources at 940-898-3759 or rgardner2@twu.edu.
